An exciting opportunity has arisen for a Heart Failure Advanced Clinical Nurse Specialist (ACNS) to join NHS Fife’s Community Cardiac Specialist Nurse team on a fixed term basis.   

Following a successful bid to the Scottish Government we have secured funding to support the reduction in heart failure related hospital admissions. 

You will:
• Review admission data with a particular focus on patients not known to our service 
• Review existing care pathways (step up/step down care, early review post discharge)
• Strengthen links with primary, secondary & community care services.
• Scope the use of IV diuretics in the community setting

You will be an experienced cardiac nurse with non medical prescribing and clinical assessment & decision making qualifications.

Working within the four pillars of advanced practice you will ensure the delivery of high quality, clinically effective care, utilising advanced clinical skills and knowledge. You will work as a member of the wider Health and Social Care Team with the ability to work autonomously utilising your own initiative, clinical judgement and decision making.  

As this is a community post you will be required to travel across Fife.

The post is for a fixed term until end of March 2024 and applications on a secondment basis will be considered providing you have prior agreement for your current line manager.
