These are exciting times within Aberdeen City’s Hospital at Home as we expand our team to further prevent avoidable hospital admissions and facilitate early discharge from hospital. Effective leadership is necessary for our Hospital at Home service to create the conditions for transformational change.

We advertise for a Team Leader who is passionate about leading a multi-disciplinary team.  The Team Leader would work collectively and be supported within an established leadership structure, being expected to develop a sustainable workforce with the skills, competencies and confidence to manage acutely unwell patients safely at home.

The successful candidate will be kind, innovative, professional, empathetic, understanding and highly motivated with the ability to motivate others.  You must be committed to working with the management and multidisciplinary team to ensure delivery of a high standard of person centred, evidence based care to patients in accordance with the NMC/HCPC code of Professional Conduct, national and local policies and guidelines.

Ideally we are looking for an experienced candidate to work 0800-1600 Monday to Friday.  While the post is full time, part time candidates may be considered. Any part time applicants wishing to be apply are asked to identify their preferred hours within their application.


The successful candidate must have access to a car for work purposes and hold a full UK driving licence as travel throughout Aberdeen city area is essential.

While the post is available on a fixed term 1 year basis, if you would wish to apply and are currently employed by NHS Grampian on a permanent contract, you must have a discussion with your manager and have a secondment agreed in principal before submitting your application.
