An exciting opportunity has arisen for a Liaison Nurse to join Fife Health and Social Care Partnerships Care Home Support and Assurance Team.

The Care Home Support and Assurance Team, led by the Head of Nursing and based within the Nursing Directorate, work in partnership with providers and other multi agencies to assure the care provided in our 75 care homes is safe, effective and person centred.

We are seeking a 1st level registered nurse who has a qualification in Adult or Mental Health and who is approachable, confident and can deal with challenging situations as well as demonstrating an appreciation of the diversity of the workload. The role includes supporting care home managers and staff with assessment, advice and referrals, as appropriate, for individual residents, as well as providing basic advice, awareness sessions and support in areas such as the fundamentals of care and essentials of psychological care. 

You will have a good knowledge and experience of a wide range of health issues gained through working across older adult clinical care settings. Experience of working in a community or care home setting is desirable, although not essential. You should be flexible in approach, with experience of multi disciplinary partnership working, leadership and an interest in working as part of a motivated and flexible team. Experience in quality improvement methodology, change management and service development would also be an advantage.

Excellent communication skills and the ability to work both individually and as part of a team are key to this post too.

You will be based either at Lynebank Hospital in Dunfermline or Stratheden Hospital in Cupar. The core hours are Monday – Friday 0830 -1630 and travel throughout Fife will be required.
