Job Purpose
As part of a multi disciplinary team the post holder will have responsibility to deliver an integrated discharge service across NHS Acute, and Primary care, liaising with colleagues in secondary care to achieve a streamlined process for patients. This encompasses assessment of care needs, working in partnership with statutory and voluntary agencies in the delivery of patient focused care packages. Communication networks developed through monitoring and appropriate intervention allowing the facilitation of shared care packages for the patient with complex health/social issues.
This post requires the post holder to have a PVG Scheme membership/ record. If the post holder is not a current PVG member for the required regulatory group (i.e. child and/or adult) then an application will need to be made to Disclosure Scotland and deemed satisfactory before they can begin in post.
