The shift pattern is 09.00-17.00.
An opportunity has arisen for a Health Care Support Worker B3 37.5 hours. The role includes working as part of the MDT to support the delivery of a specific care plan to the patients within the NE Older Peoples' Mental Health Team as implemented by registered colleagues. This will require home visits and lone working. The role will support the Community Psychiatric Nursing staff within the Multi-Disciplinary Memory Clinic to deliver comprehensive, patient centred assessments, delivered in a compassionate manner.
The role will support the delivery of the physical health care policy in line with GG&C policy and the successful candidate will participate in the CMHT Desk duty rota.
