This is a Fixed term post for 3 years.
A new exciting opportunity has arisen for a Cancer Patient Pathway Co-ordinator to join our Colorectal Cancer Nurse Specialist Team within this specialty of South Sector General Surgery Glasgow. The Cancer Patient pathway Co-ordinator will be responsible to the Clinical Nurse Specialist Team lead but should be able to work autonomously acting as a point of contact providing support from diagnosis through the whole cancer journey. You will triage incoming calls and ensure ongoing appropriate referral or escalation of care is made. You will also work collaboratively within the National Colorectal Cancer Service (MCN Colorectal cancer) ensuring patients meet Scottish Government target treatment times and that they receive the right care at the right time every time.
You will be able to organise your own time and work to an agreed job plan liaising with the cancer multidisciplinary team in ensuring the quality measures are met and improvements made in the patient's cancer journey. A fundamental part of this role is ensuring good communication between all members of the Multidisciplinary team. Key IT skills are essential and experience with Trackcare, clinical portal is desirable. A qualification and/or experience in healthcare provision is preferred for this role. You will be working Monday to Friday (days) and membership of Disclosure PVG scheme is required for this role.
