We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.  

This is an exciting opportunity for an innovative and experienced rehab physiotherapist to join a new and evolving frailty service, Home First Response Service within the community rehabilitation team. This service will be based within the Rehab team in Renfrewshire and alongside the wider MDT and will be providing assessment and care to people living with frailty in the community. The community team will interface closely with our acute based team to support safe discharges and work proactively with wider community teams and services to enable people to live at home whom are frailty positive and to prevent unnecessary admissions.

Hours of work will be flexible to cover between 9am and 8pm, 7 days per week. Initially working pattern will be 9am to 5pm Monday-Friday. It is expected that they hours of shift pattern will extend in the future as the service develops. Referrals to the team will be from acute sites to aid a speedy and safe discharge from GP and primary care with the aim of keeping people at home by the provision of extra care and treatment.
