The shift pattern is Monday-Friday 08.30-16.30.
A vacancy has become available for an RSW Band 3 within the East Dunbartonshire Community Rehab Team. The multidisciplinary team aims to work with individuals, in their home environment, to identify their goals and priorities and work with them to improve independence, function and mobility. Patients have a diverse nature and wide range of conditions, from newly diagnosed conditions, to end stage palliative care.
The post holder will work with clinicians within the speciality, taking responsibility for an identified caseload, under the Clinical supervision of the Team. The post holder will visit people in their own homes and in the Community, and will work towards the patient’s Rehabilitative goals, as agreed with the Assessing Clinicians. Walking Aid assessments may be carried out with patients and close relationships maintained with assessing Clinical staff.
The role will involve working in partnership with clients, carers, Health and Social Work staff, residential care staff and Independent Sector Agencies to ensure optimal care. You will have a key role in the Team of supporting individuals in their recovery and helping to maximise their independence.
It is essential you have some experience of working in a community setting and will be happy to comply with the Code of Conduct for Health Care Support Workers and have familiarity with the Code.
A full driver’s licence is essential.
