This is a 12 months fixed term post.
The shift pattern is, 08.30-16.30 5/7 days including weekends.
This is an excellent opportunity for 1.0 WTE (12- month fixed term contract or secondment ) for a Band 5 Care Home Support Nurse to join a highly motivated and dynamic team working across role and organizational boundaries. The CHSN will provide an accessible, appropriate, high quality, culturally sensitive and effective nursing service to support staff and residents in the residential care home setting, by working in partnership with service users, care providers and other agencies. Based within Renfrewshire HSCP, the CHSN will be working alongside ANPs, PDN and CHLN's within the Care Home Support Team, GP practices with Care Homes to provide assessment and care for the patients registered within these practices. The CHSN will complement core services provided by the HSCP and will also work alongside community nursing and rehabilitation services.
It is essential you are a Registered Nurse on the NMC Register. Significant comprehensive post registration experience within a healthcare setting is desirable. The shift pattern is 5 days over 7
