Post Title: Community Diabetes Health Care Support Worker

Hours: 18.3 Hours ( to be worked 5.5hrs Monday and Tuesday and 7.5hrs Wednesday)
An exciting opportunity has arisen for a HCSW band 3 to join the Community Diabetes Specialist Nurse Service. The main focus of the team is supporting patients with complex Type 2 diabetes who are already on or are requiring insulin commencement. We support and educate them to self-manage their condition whenever possible. We aim prevent unnecessary hospital admission and facilitate early supported discharge where possible, enabling patients to live long healthier lives.
You will work under the direction of the Clinical Nurse specialists and Assistant Practitioner. You will be responsible for patient referrals for Continuous blood glucose monitoring (CGM) as well as home visits to support patients who are housebound and require support to access “Nearme” video consultation, or are having issues with injection technique, insulin injection technique, or blood glucose monitoring.
You will also support the Clinical Nurse Specialists and Assistant Practitioner to deliver patient education groups.
Previous experience working within the diabetes speciality or Community Nursing setting is desirable but not essential as full training will be given.
The successful candidate will have:
• Excellent organisational skills, effective communication and interpersonal skills.
• Be a team player and have a genuine interest in working within the Community Diabetes service.
• Relevant HCSW SVQs level 2 and 3 in healthcare.
• The ability to effectively and reliably travel to various sites and patients homes across Fife.
• You will be committed to undertaking all training necessary for the role.
In return our service offers regular supervision and supports training opportunities. You will be allocated a mentor and supported to achieve the national diabetes competencies for non -registrants.

NHS Scotland is reducing their full-time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay. This reduction will also be applied pro rata for part time staff.
