NHS Forth Valley Pharmacy Services have an exciting opportunity to recruit an enthusiastic, forward thinking pharmacist to their team of pharmacists working across the Community Hospitals.
The team is currently undertaking new and innovative roles as part of partnership working across Community Hospitals, thereby fulfilling the vision set out by Achieving Excellence.
You will have a proven track-record in values-based and clinical pharmacy leadership, and be committed to integration, collaboration and partnership working, demonstrating outstanding people and engagement skills.
This post provides a unique opportunity for a highly motivated Pharmacist to focus on work within the community setting. The post-holder will work closely with multi-disciplinary teams to develop pharmaceutical care for a wide range of people and liaise closely with care at home services, hospital, and community pharmacy colleagues to ensure seamless pharmaceutical care.
You will also support wider pharmacy developments including non medical prescribing, attend multidisciplinary team meetings and undertake polypharmacy reviews. Part of this role will involve supporting community pharmacies to implement local and national services and supporting integration at the many interfaces between care settings.
Ability and willingness to travel throughout the area of service delivery is essential for this post.
