NHS Scotland is reducing their full time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay.  This reduction will also be applied pro rata for part time staff.  This advert and any subsequent offer/contract of employment therefore reflects the new working hours.  However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.  If you have any questions or concerns please contact the Recruiting Board.


The shift pattern for this position is 08.30 -16.30 hours worked 5 over 7 per week inc. weekends/public holidays.

Role of the department: Caring is fundamental to nursing services and to all nursing functions. The core values of nursing must underpin the practice of every nurse and drive care that promotes positive and equitable engagement with patients, families and carers as the central focus of practice. Therefore, the District Nursing Service operates an open access policy delivering care to patients within either a community or primary care setting, or who have a nursing need which makes a home visit more appropriate, the service operates over 7 days.

We are looking for a dynamic, caring and motivated nurse with a proven track record of community nursing experience and who also has Long Term Condition experience either holding their LTC certificate or working towards attaining the qualification.

The post holder will be the keyworker for patients within integrated teams who require additional clinical support and management of their Long Term condition.

The job the applicant will be doing: 

As an autonomous practitioner the post holder will be responsible for the assessment and treatment of patients with complex health and social care needs. Ensuring provision of a holistic nursing assessment and high quality, safe, evidence-based care to patients and carers with the aim of maintaining patients in their own homes.

The post holder will work collaboratively with Community Nurses, Practice Nurses, GP’s, In Reach and Discharge nurses managing patients with chronic disease and frailty through autonomous practice.

The post holder will work in this adaptable and evolving role, focusing on safe, effective admission avoidance and supporting timely discharge from acute hospitals, with a whole systems approach.

The post holder will co-ordinate care and strive to reduce the risk of acute re-admissions by identifying risks, implementing management plans and collaborating with the whole community resource to facilitate ongoing care management.

Skills / Qualifications:

A 1st level registered nurse with significant post registration and community experience is essential.

An interest in Palliative Care and Management of Long Term Conditions / Anticipatory Care alongside associated technical skill & ability is considered advantageous. E.g. enteral feeding T34 Syringe Pumps
