The shift pattern for this position is to be confirmed (Please note the salary is Pro Rata) for part time hours.
An exciting opportunity has arisen for an enthusiastic, forward thinking individual to join our service in the role of Discharge coordinator for Inverclyde Mental Health Services. This is a 25hr fixed term post for 12 months and the post holder will work 5/7, with flexibility to vary shift patterns dependent on service requirements.
The post holder will be responsible for the direct line management of an integrated discharge coordination team between health and social care within Inverclyde HSCP. A key role is managing and coordinating patient care specifically in relation to timely discharge from mental health inpatient beds across the mental health campus. The post holder will support the strategic planning and delivery of this new and developing post.
The post holder will have overall responsibility for monitoring activity related key performance indicators and has lead responsibility for clinical governance activity and monitoring which includes care assurance; standards compliance; professional regulation; responding to complaints; staff governance; Clinical Supervision; Line Management Supervision; legislative frameworks; national and board policy/ guidelines implementation.
The post holder will work alongside both community and in-patient teams to ensure a positive experience of discharge for patients and their carers.
It is essential that you are a Registered Mental Health Nurse with the NMC and have robust experience as a Band 6 nurse.
