We are delighted to announce an exciting opportunity for a fixed term/secondment post up to 12 months (maternity cover) for the Senior Clinical Pharmacist role within NHS Fife with a focus on frailty and elderly care. 

With an ageing population with more complex needs, we are looking for an experienced candidate to lead the transformation and integration of the clinical pharmacy service to frail and elderly patients across NHS Fife. 

As a clinical practitioner, the post holder will work closely with the wider MDT to make patient-centred prescribing decisions relating to frail and elderly patients, in order to contribute to: medicines optimisation; reduction of falls and readmissions; enhanced discharge planning and continuity of medicines safety and governance. 

This is a fantastic opportunity for an independent pharmacist prescriber (qualified or in training) to realise the benefits of undertaking patient-centred medication reviews in this cohort of patients, improving patient care and reducing harm from medicines. 

As a key member of the Community Health Pharmacy team, the post holder will predominantly be based at Victoria Hospital Kirkcaldy, but will have oversight for clinical pharmacy provision for the frail and elderly population across NHS Fife.  They will support this patient cohort based on patient need and demands on the service, including frail and elderly patients in Community Hospitals. They will work to develop seamless care pathways when patients move between care settings and provide leadership to the Specialist Clinical Pharmacists rotating through this area. 

The NHS Fife Pharmacy Service is undergoing an ambitious programme of transformation and integration. It serves a population of approximately 380,000 people and is provided by an integrated team of around 300 Pharmacy staff, including Pharmacists, Pharmacy Technicians, Support Workers and Administrators. The team work across Acute and Community hospital sites, General Practices, Mental Health services, and a range of specialist teams. Partnership working is at the core of our values, and we work closely with other members of the multi-disciplinary team, including our Community Pharmacy colleagues, to deliver the highest quality care for everyone in Fife.

Please note the job description is for a generic senior clinical pharmacist role and a detailed job plan and annual objectives will be agreed in line with directorate strategic priorities.

A requirement of this post is to become a member of the Protection of Vulnerable Groups (PVG) Scheme Prior to appointment.

NHS Scotland is reducing their full-time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay.  This reduction will also be applied pro rata for part time staff.  Your offer/contract of employment will therefore reflect the new working hours.  However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.  If you have any questions or concerns, please contact your Hiring Manager.
