NHS Greater Glasgow and Clyde is one of the largest healthcare systems in the UK employing around 40,000 staff in a wide range of clinical and non-clinical professions and job roles. NHS Scotland is reducing their full time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay. This reduction will also be applied pro rata for part time staff. This advert and any subsequent offer/contract of employment therefore reflects the new working hours. However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week. The shift pattern for this position is 5 days over 7 including weekends.
An exciting opportunity has arisen to work within the Care Home Liaison Nursing service within East Renfrewshire HSCP for a 12 month fixed term contract 22.5 hrs per week 5 days over 7 including weekend working as part of a rota.
The post holder will use a collaborative and co-ordinated approach to provide enhanced support and clinical advice to care home staff to enable high quality standards of care to residents with a focus on prevention of avoidable hospital admissions.
The CHLN will work in partnership with the MDT team and Care Home to ensure safe and effective care using a quality improvement approach to improving outcomes for residents.
The ideal candidate will hold an adult nursing registration with considerable relevant experience and transferable skills and will have or be willing to undertake further professional development related to the role.
Working in partnership with Social and care home staff the post holder will support General Practitioners and other colleagues in the ongoing provision of enhanced services for residents within care homes.
The ideal candidate will have a minimum 4 years post registration experience, preferable but not essential within Primary Care be a non-medical prescriber V150/V300. Educated to degree level.
Driving license holder/driving is essential to this post.
