An exciting vacancy has arisen within our department to manage the Care at Home team which functions as part of the District Health and Social Care Integrated Team. The Care at Home service enables people to realise their potential through the ethos of Reablement therefore maximising independence and supporting the individual to remain in their own home. It is the post-holders’ responsibility to meet the set objectives targets and person-centred outcome measures for the care at home service and individual practitioners.

To create care plans from multi-disciplinary assessments and coordinate and develop care packages for service users with complex needs identifying clear outcomes, monitoring and reviewing care packages every 6 months or as required.

To work within Self Directed Support (SDS) Legislation and deliver a person-centred Care at Home service to meet the assessed need and personal outcomes of those referred to the service in line with the standards required by the Scottish Social Services Council (SSSC) of a registered Care at Home service.

NHS Scotland is reducing their full time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay.  This reduction will also be applied pro rata for part time staff.  This advert and any subsequent offer/contract of employment therefore reflects the new working hours.  However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.  If you have any questions or concerns please contact the Recruiting Board.
