We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.


The shift pattern for this position 9am – 5pm Monday to Thursday with 30 mins per week to be taken on rota basis.

Requirements:

  • The post-holder is expected to be a Registered Mental Nurse (Level 3 or 13 on NMC Register) or equivalent with a minimum of 5 years post registration experience.
  • They are required to have a good working knowledge of mental health problems
  • Be efficient in carrying out mental health assessments to support care planning and risk management.
  • Be self-motivated with the ability to work autonomously and have the ability to consider individual needs while considering the emotional, psychological and social aspects to patient care.

Duties include:

  • The community wellbeing nurse will be involved in clinical audit and activities to promote innovative practice with statutory and non-statutory agencies.
  • Work with Primary Care, Community Mental Health Teams, In Patient services and other mental Health Network services and agencies to support patient care and will use a “stepped / matched care” model to support practice and decision making.
  • Work in partnership with patients, families and carers, to support the management of mental health needs and will be governed by the principle of the delivery of the minimum effective level of intervention.
  • Be responsible for improving patient care at the point of initial contact, improving access to services and liaison between services within local areas.
