The shift pattern is 32 hours over Monday-Friday 8.30- 4.30 with potential for weekend work in the future.

Please note the salary is Pro Rata for part time hours. 

The Community Respiratory Team is a dynamic and responsive multidisciplinary team (MDT) that aims to support patients with moderate to severe COPD within Glasgow City in their own homes. The team is comprised of Physiotherapists, Nurses, OT, Pharmacist, Dietitian, Rehabilitation Support Worker, Business Support and Leadership roles. It is an evidence based service with the MDT supporting three distinct intervention pathways:

• Prevention of hospital admission (support for acute exacerbation)

• Early supported discharge

• Chronic disease management (rehabilitation and self-management)

The post holder will act independently and use their own initiative within the bounds of existing knowledge and skills. You will work autonomously within their scope of practice to make clinical judgements regarding patient prioritisation and management. Work is triaged and allocating depending on clinical patients need and the post holder will lead regularly in triage decisions, referral allocation and will coordinate Consultant lead MDTs and safety huddles. As a senior and advanced specialist team member, the post holder will deputise for the clinical lead or team lead on a regularly basis. You will link with the Respiratory Early Supported Discharge (ESD) team, and hospital based respiratory assessment services/respiratory nurse specialists.

It is essential that you have a First Level Registration on appropriate adult nursing part of the NMC register. You should also have achieved or are working towards your non-medical prescribing qualification. Advanced assessment and decision making at Masters level or evidence of advanced specialist decision making skills within experience or professional development is also essential.
