Aberdeen City Out of Hours Nursing Band 6 Opportunity
There is an opportunity for Band 6 Senior Community Nurses to join our Out of Hours Community Nursing Unscheduled Care Team within Aberdeen City HSCP.
The OOH Senior Community Nurse works with and supports Band 5 and Band 3 colleagues delivering a person centred clinical nursing service, which prioritises end of life care and symptom control for patients in their own homes and plays a vital role in supporting admission avoidance. The role includes triage and management of unscheduled nursing referrals from GMeds, NHS 24, Hospital at Home and other professionals, as well as support for the District Nurse with team management responsibilities.
Candidates must possess a Graduate Diploma in Integrated Community Nursing, or a relevant Post Graduate Certificate and evidence current clinical experience in a Community Nursing role at Band 6, which includes triaging of referrals, workload planning, palliative and end of life care.
Hours: 1800-0000 3 shifts ON / 5 shifts OFF within an 8 day rota (16 hours per week pro rata).
Car owner/driver and current UK driving license is required. Car must be LEZ compliant.
