An exciting opportunity has arisen for an innovative individual to provide maternity leave cover as a Patient Navigator within the Upper GI and Hepato pancreato bilary (HPB) cancer nursing service, within the Surgical Directorate. We are seeking an individual who will embrace this exciting and rewarding role.
You will be an integral part of the UGI and HPB cancer nursing team and will assist, support and co-ordinate the pathway for their patients. As a patient navigator in this service, you will be involved with patients on a suspected or confirmed cancer pathway, supporting them throughout their cancer pathway by maintaining high patient experience and outcomes.
You will ensure the efficient channelling and close coordination of the patient pathway ensuring robust communication between patients, relatives, nurses and oncology teams. You will signpost patients and carers to the relevant source of support and information appropriate to their needs.
You will demonstrate excellent organisational skills, be flexible in approach, able to exercise initiative and demonstrate a consistently high standard of professionalism, being aware of the need for confidentiality and integrity.

Please note the salary for this post is pro rata to part time hours.
