The Home First Response Service is a new service focused on working differently and collaboratively with acute and community teams to identify frailty and support people home or within a homely setting. As part of the team we are looking for an innovative and experienced physiotherapist to join our team within the RAH. This post is a community inreach post with professional supervision supported by established structures within Renfrewshire HSCP. The team focus is towards the front door of the hospital and by working alongside colleagues in ED, assessment units and as part of the wider frailty services within the RAH seeks to improve people’s outcomes by utilising community interventions to enable discharge home quickly. As this is a community post, it will be managed and supervised under the Renfrewshire HSCP management structure.
