Band 3 14.1 hours per week. 7 day working with a mix of daytime shift patterns from 8am until 8.30pm.

We are looking for an energetic and innovative health care support worker to join our Home First service. If you are passionate about supporting people to live as well as they can do at home following their discharge from hospital and are motivated to help people avoid admission to hospital by helping them to be as independent as possible, then get in touch.

You will be part of a team of Health Care Support Workers, Occupational Therapists and Physiotherapists and you will visit people in their own homes to help them with their daily tasks and carry out Reablement Programmes from the Physiotherapist and Occupational therapist. The emphasis of this role is to help people to do as much as they can for themselves so that they can stay active and live as independently as possible.

The work will be varied and fast paced, you will work independently and usually visit people on your own. If you enjoy working with people to help them to recover from injury or illness and like working as part of a team, then this job may be for you!

The post holder will be based primarily in Central locality team covering Central/Tweeddale area and must be able to drive and access to a car.
