The post holder will support and supervise a small group of workers within a defined geographical area on a day to day basis assisting multi-disciplinary staff and Enablement Officers within the District Health and Social Care Integrated Team in the delivery of high standard enablement support to individuals and their families/carers.
The focus will be on maximising independence and supporting the individual to remain in their own home. Undertake simple reviews of enablement plans. Support enablement workers in adjusting and adapting enablement plans where problems have been identified /needs of the client have changed eg washing and dressing, mobility.
The post-holder will support and enable individuals to achieve agreed goals through an enablement approach. The post-holder will work within the team under direction and instruction from the enablement officer. The post-holder is expected to independently manage a designated case load. It is expected that the post-holder will work unsupervised on delegated duties with access to advice and support as required from the Enablement Officer and the relevant integrated team lead professional.
Full driving licence and access to own vehicle required and registration with the Scottish Social Services Council required within 6 months of commencing in post.
