This is an exciting opportunity for an innovative and experienced occupational therapist to join Home First Response Service a hub and spoke model with a team in the acute site and community. It is a fixed term contract of 9 months to cover maternity leave. This post is based within the Royal Alexandra Hospital working alongside the wider MDT will be providing assessment and care to people living with frailty. The acute based community inreach team will interface closely with our community based team to support safe discharges and work proactively with wider community teams and services to enable people to live at home and prevent unnecessary admissions. Hours of work will be flexible to cover between 8am and 8pm, 7 days per week. Initially working pattern will be core hours over 3 days per week. The service currently runs Monday to Friday. Expectation that the hours and shift pattern will extend in the near future as the service develops. Referrals to the team will be from front door areas to aid a speedy and safe discharge, aim of keeping people at home by the provision of extra care
