Clinical Development Fellow – Hospital at Home
Full Time or Part time – Fixed term for 12 months
Salary Scale - £45,504 – £71,550 per annum, pro rata if part time
Are you looking for more experience in managing unwell older people at home? Then this post is for you. We have a well established Hospital at Home (H@H) service within NHS Forth Valley that is looking to expand its medical team with the addition of a clinical development post. This will be a fixed term post for 1 year.
These posts are aimed at CMT / FY2+ level i.e. those doctors having recently completed the UK Foundation programme and who are looking for some experience of caring for frail older people at home. It would ideally suit those looking at a career in general practice or within wider geriatric medicine services and who wish to develop and enhance their quality improvement skills. They will be supervised by a team of senior geriatric medicine consultants, a specialty doctor and General Practitioners with a special interest in H@H
The clinical development component combined with supervised and directed activity will allow for 8 hours development per week. 4 of these hours will be for continuing personal and professional development including structured departmental and directorate teaching. The remaining 4 hours will be linked to the departmental service development priorities.
This post is designed to compliment already existing services while aligning with the departmental strategy to improve the care of frail older people within Forth Valley.
The Hospital at Home service provides a short term targeted intervention that:
- Provides acute, hospital level care by multidisciplinary health care professionals in an individuals’ home or homely setting that would otherwise require acute hospital admission.
- Is able to assess, investigate and treat patients in their homes at a level of care equitable to an acute hospital setting following agreed protocols and pathways.
- Is delivered by a highly skilled, multidisciplinary integrated team care in collaboration with multiple existing agencies including primary care, health and social care partnerships and third sector.
- Provides a 7 day a week service from 0800-2000 throughout all of Forth Valley, where patients will be assessed and risk assessed within 2 hours of referral to the service and will receive daily multi- disciplinary health care professional assessment and treatment within their own home.
- Interfaces with existing well developed community services, reducing unwarranted variation thus delivering high quality, cost effective care.
