The shift pattern for this post is Monday to Friday between 08:00 and 16:00

Please note the salary is pro rata for part time hours.

This is an exciting opportunity to join the South Glasgow Diabetes service and to develop as an independent practitioner in a Band 6 Clinical Nurse Specialist role. In application, you will have relevant registered nurse experience and have an interest in training and supporting patients with Diabetes to self manage their condition

You will act as a resource for expert advice and training, whilst facilitating a point of contact for patient consultation. You will support a clinical caseload, plan efficient outpatient care & independent follow up reviews to support physical and mental wellbeing. As a nurse specialist you will provide patient/ family education in an outpatient setting.

You must have the ability to work independently and possess a high level of interpersonal and confident communication skills to support this specific client group whilst managing telephone consultations, patient referrals, interface with hospital clinicians and general practitioners to maintain excellent clinical links between the services, whilst upholding confidentiality at all times. You will independently assess and treat patients in an Outpatient setting.

You will be responsible for patient education, participation in service development in conjunction with the team lead and maintaining service objectives to meet the changing demands of delivering specialised care to patients with Diabetes. You will be expected to contribute professionally within groups which may cross-traditional or service boundaries and possess a high level of ITC skills including e- forms, Trakcare and portal. All Statutory / Mandatory Learnpro training modules must be compliant and include educational modules relevant to this specialty.

The successful candidate will be based in South Sector, Greater Glasgow & Clyde.
