The Hospital at Home (H@H) service based at Lorn and the Isles Hospital covers the Oban and Lorn area. It is a developing service that has proven to be a safe alternative to hospital admission. The service provides acute care at home with the objective of reducing hospital admission and reducing length of stay in hospital through expanding clinical pathways.
The Nurse Practitioner within the H@H service is a key role in the management of acute and complex patients; providing a high level of complex decision making within a multidisciplinary team. Management of patients with both acute and long term conditions across a number of different clinical pathways, where their medical conditions can be managed safely and effectively at home.
We are looking for a nurse practitioner post to expand our team to enable a service across 7 days. The post could be fixed term or a secondment opportunity until March 2026. This could be a development post that would involve undertaking educational modules with mentorship support.
