An exciting opportunity has arisen for a Health Care Support Worker to join the Community Treatment and Care Team. The team is based within various locations across Angus.
Working within a team you will provide care and support to patients and support to their carers within a clinic setting.
Angus Health and Social Care Partnership is in the implementation phase of Transforming Primary Care Services and part of this new work is to develop and embed a new model to provide treatment room care for patients in Angus. You will have a desire to work in the community, be well organised and have excellent communication skills, as well as experience of working within a health or social care setting. This role will involve working people of all ages including children. Please see job description and person specification for other requirements.
You will have the ability to work on your own initiative and referring appropriately to the registered professional. Ability and willingness to travel throughout the area of service delivery is essential for this post to provide patient care.
The duties of this post require the successful candidate to be a member of the Protecting Vulnerable Groups (PVG) Scheme. More information on this scheme can be found at www.disclosurescotland.co.uk
