Discharge Co-ordinator – Band 6
Salary Scale: £41,608 - £50,702 per annum, pro rata
PERMANENT/PART TIME
An exciting opportunity has arisen within Perth & Kinross Health & Social Care Partnership for a Band 6 Registered Nurse or AHP equivalent within the Discharge Hub based in Perth Royal Infirmary.
The Discharge Team lead and co-ordinate complex discharge planning as an integrated multi-disciplinary team allowing for safe, effective and timely discharges in order to maintain capacity & flow. Person centred assessments are carried out which address health and social care needs to support and facilitate discharge planning across 7 days. You will work alongside multi-disciplinary and multi-agency teams across traditional, professional and service boundaries with the ability to adapt and manage change and promote joint working.
You will show continuous professional development, be able to demonstrate leadership and excellent communication and interpersonal skills.
Candidates should have recent experience in assessment of older people. You must have the ability to make high level clinical judgements and have experience in problem solving/decision making in order to effectively manage patients’ health and social care needs.
There will be an expectation that the post holder will actively participate in creatively developing the service.
You must be registered on Sub-part 1: RN1 or RNA or Sub-part 2: RN2 of the UK NMC Register
