Position:
An exciting opportunity has arisen for a Band 6 within our Cardiac Rehab/ Healthy and Active Rehabilitation Team based at University Hospital Ayr.
Hours and Shift Pattern:
29.5 hours split evenly over Monday to Thursday
This post is offered on a Permanent basis.
What you will do:
This post will require a skilled practitioner to lead changes that widen the treatment options available specifically for with long term conditions.
You will provide direct clinical care and treatment to people affected by cardiac and multiple long term conditions including post viral symptoms. The post holder will plan and provide patient education sessions, deliver safe group exercise and contribute to the improvement of the Healthy and Active Rehabilitation Programme (HARP) as a whole. The majority of time will be spent in an outpatient setting and you will be expected to lead a number of hospital and non-hospital based classes to a variety of patients, some of which will be high risk, frail and complex. You will be expected to work in a clinic and a class situation both independently and with other members of the multi-disciplinary team. You will also contribute to the work of a team where an increasing number of service users have multi-morbidities and should have experience and confidence in this area. We are also committed to trialling digital service improvement and new developments that contribute choice to our service users.
You will have experience of assisting people to achieve their goals, and be passionate about supporting self-management to help people live well with their long term conditions.
The job can involve travel throughout Ayrshire, across all hospital sites.

Knowledge, training, qualifications and/or experience required to do the job:
As a clinician with skills within a rehabilitation team, you will have experience in assessment, evaluating patient progress and planning treatment programmes that are individualised and meet the needs of each person. The post will involve a “what matters to you” type approach using personal goal setting, during all stages of the patient journey.
Experience of leading groups is highly desirable.
