The shift pattern for this post is The shift pattern is 5 over 7 days including Out of Hours period.

An exciting opportunity has arisen within Glasgow City HSCP for an Advanced Nurse Practitioner (ANP).
The Glasgow City Hospital at Home service provides acute-level care within patients’ own homes as an alternative to hospital admission. The service manages patients with acute or exacerbated medical conditions – particularly respiratory illnesses. By delivering early assessment, diagnostics and evidence-based interventions in the community, the service aims to reduce unscheduled hospital admissions, improve patient experience and support timely discharge.

This approach helps to alleviate pressure on inpatient bed capacity and reduces emergency departments waiting times across Glasgow City.
The post will provide clinical, operational and professional leadership. You will be a registered nurse educated to masters level in advanced practice that will enable competence in clinical assessment, clinical reasoning, judgement and diagnostic decision making, anatomy and path physiology, non medical prescribing (V300), leading delivery and evaluation of care. You will have significant experience either within acute or community working as an ANP.

This post will be integral to the development of the Hospital at Home Service

The right candidate will be able to work across disciplines and teams and have a proven track record of service development and leading a team.
