We have an exciting opportunity for enthusiastic Support Workers to join our Discharge to Assess (D2A) Team in Moray. This is your chance to be part of a therapy-led, patient-focused service that helps people return home safely and regain independence after hospital discharge.

What We’re Looking For
- Experience in hospital or community care settings as a Support Worker
- Interest in therapy and rehabilitation.
- Willingness to learn and develop through training.
- A caring, proactive approach to supporting patients.
What You’ll Do
- Deliver Occupational Therapy and Physiotherapy programmes under supervision.
- Help patients achieve independence in daily activities.
- Promote health, wellbeing, and self-management.

As from 1/4/26, the Agenda for Change full-time working week will be reducing from 37 to 36 hours per week. Part time hours will be reduced pro-rata. However, there will be a corresponding increase in the hourly rate so that pay will not be affected.
