The role:
NHSGGC (2016) Community Mental Health Team (CMHT) Operational Framework defines the CMHT as forming part of a whole system approach to mental health services for the adult and older adult population. The service is delivered in conjunction with Primary Care Mental Health Teams (PCMHT); Acute Services (Crisis / Home Treatment Services & out of hours (OOH) Services); specialist Mental Health services and a range of statutory and non-statutory services that support the delivery of care.
CMHTs function using an integrated multi-disciplinary approach and aims to:
- work with people with complex mental health problems and associated risks who typically require longer-term care and treatment, multidisciplinary engagement, and with the capacity to provide assertive care and to manage higher levels of risk .
- work in partnership with patients, families and carers, primary care and other agencies to design, implement and oversee comprehensive packages of health and social care where needed, to support people with complex mental health needs.
- Deliver services in a suitable environment that is accessible to the individual (and their carers). This will include team base, GP surgery, the patients’ home or other appropriate community setting.
Interventions delivered by the CMHTs will be on a “stepped / matched care” basis, based on individualised assessment of need, and will be governed by the principle of the delivery of the minimum effective level of intervention.
All patients being cared for have a Named Nurse and/or Keyworker who is tasked with working closely with their identified patients/carers to develop a participative, therapeutic relationship and deliver high quality mental health care in an environment where patients feel valued and where confidentiality is respected.

Shift Pattern - Monday -Thursday, 9am-5pm Friday, 9am – 4.30pm
Key responsibilities:
Acting as Named Nurse/keyworker for an allocated caseload within the multidisciplinary team.
Completing holistic assessments, including risk assessment, and planning, delivering, and evaluating care.
Providing appropriate nursing care in the least restrictive and least disruptive way.
Identifying and addressing patients’ health‑promotion needs to support mental and physical wellbeing.
Recognising and responding to vulnerabilities, including issues related to gender, health, domestic abuse, child protection, and self‑harm.
Building therapeutic relationships with patients and carers, ensuring inclusive, respectful, culturally sensitive care and effective communication with other services.
Delivering evidence‑based psychological therapies or approaches within agreed competency levels.
Administering prescribed medication/treatments, monitoring effectiveness, and managing side effects.
Working collaboratively with patients and carers to deliver person‑centred, values‑based care and support relapse prevention.
Ensuring all practice (including delegated tasks) complies with the NMC Code and relevant legislation (MHA, AWI, ASP, CYPA).
Supporting clinical governance through effective communication, leadership, policy adherence, audit, evidence‑based practice, learning culture, research involvement, and risk‑reduction strategies.
Participating in caseload/line management supervision and performance appraisal, including PDP development.
Engaging in regular clinical supervision and reflective practice.
Monitoring and supporting the practice of Healthcare Support Workers and junior staff.
Providing line‑management supervision and performance appraisal for junior staff and supporting their development.
Supporting and supervising learners, including student nurses, and contributing to a high‑quality learning environment.
Identifying and addressing personal development needs to ensure safe, effective, person‑centred care (including Flying Start® for new registrants).
Liaising with statutory and non‑statutory agencies across hospital and community settings, particularly at health and social care interfaces.
Knowledge, training, qualifications and/or experience required to do the job:
Essential
- Registered Mental Health Nurse - level 1 NMC registration
- Working knowledge and application of mental health nursing care (including physical healthcare)
- Evidence of active and ongoing CPD
- Evidence of Clinical Supervision/ reflective practice
- Working knowledge of mental health / mental illness/ wellness and recovery
- Working knowledge of rights, values based practice and legislation frameworks
- Evidence of using digital technology/ information systems/ email and basic applications
- Can evidence in career history, the application of a range of skills and behaviours e.g., Organisational skills, Self-directing and Self-motivating, Effective written and verbal communication skills Including negotiation, influencing, persuading, facilitating and conflict resolution skills, Decision making skills
- Ability to work flexibly and to prioritise workload and adapt to meet person centered care without direct supervision
- Ability to form therapeutic professional relationships with others and work as a member of the wider team
- Working knowledge and demonstrable evidence of the NMC Code
- Ability to undertake a number of clinical contacts in a home/ near to home environment on a daily basis to meet which may involve driving
Desirable
- Educated to or actively working towards minimum degree level/ or equivalent
- Completion of an evidence based psychological therapies/ psychological approaches to care programme
- Completion of a mentor preparation programme/ registered mentor
- CMHN Career Development Framework evidence of pillars of practice at Level 5 Practice (equivalent): Clinical practice – safe effective person centred care, Facilitation of Learning, Leadership - applied, Research and development
- Experience of working in the community environment
