The Role

The post holder will deliver high quality care in accordance with guidance given and work delegated by registered nurses.

The post holder will ensure a person centred approach to care delivery which is safe and effective to ensure and maintain quality strategy.

The working pattern for this role is within a 24/7 service so shifts will be varied across 7 days and all hours. Flexible working is required hours including weekends, evenings and overnight.

In this key role, you will:

  • Following appropriate training, the post holder will carry out tasks as delegated by a registered nurse, e.g. blood pressure, phlebotomy.
  • Support patients, relatives and carers in order to achieve optimum health and person centred outcomes
  • Record and report patient observations in care plans to ensure maintenance of accurate and up to date records and referring to registered nurse when appropriate.
  • Act in a way that reflects the high standard expected of the community nursing team.

What You'll Bring:

  • SVQ III in healthcare, or working towards, or equivalent experience working within a caring environment/role.
  • Ability to work with people and as part of a multidisciplinary/agency team.
  • Effective written and verbal communication skills.
  • Ability to carry out assigned patient care tasks effectively within a busy environment.
  • Ability to work unsupervised.
  • To actively participate in the Personal Development Plan process to fulfil the requirements of continuing professional development.
  • Working knowledge of basic information technology

A driving licence is required; this must be a full UK/EU/EEA licence.

From 1 April 2026, the Agenda for Change full-time working week will reduce from 37 hours to 36 hours. Part-time working hours will be reduced on a pro-rata basis. A corresponding increase in the hourly rate will apply, ensuring that overall pay remains unchanged.
