The role:

The post holder will work collaboratively with the Community Treatment & Care services and assist in the implementation and evaluation of programmes of care for patients and their carers, under the supervision of the Community Treatment Room Nursing Staff/ District Nurse or their deputy. The post holder will also have a responsibility also for the delivery of a Community Treatment & Care service which includes Phlebotomy service to residents of Renfrewshire within a clinic environment and within their own homes To contribute to health improvement initiatives to individuals and families on the caseload and to the wider community. Required to work flexibly in terms of location to support other areas of team working within the CTAC / DN service.


Duration, Location, and Working Pattern:

  • The post holder will work collaboratively under the remit of the Renfrewshire Community Treatment & Care services (CTAC)
  • the shift pattern for this post is Monday to Friday 08:15 to 16:15


Key responsibilities:

  • To assist with the nursing care of the patient under the direction and instruction of a trained nurse. This will include all basic care within the patient’s home and in clinics. Including skin care, application of prescribed creams and eye drops, prompt patients to take prescribed medication; and undertakes non complex dressings
  • To provide care and support to patients and their families in their own homes and within clinics, often working alone
  • To observe and report any change in the condition of the patient and communicates this verbally
  • to the nurse in charge. Changes must be documented in patient records
  • To administer bolus feed via a gastrostomy tube and commence / discontinue PEG feeding systems as directed by trained nurse. To change the water in a gastrostomy tube in adults
  • To participate and support the care of patients ensuring skin integrity is maintained, adhering to current manual handling procedures and assists those patients unable to self-care with personal hygiene and dressing
  • To support patients and carers with catheter management and stoma care and maintenance. Encourages and supports self care for patients when assessed as appropriate
  • To carry out investigatory procedures and obtains specimens as required e.g. urine collections, stool samples
  • To remove sutures and medical clips once approved training and competency achieved
  • To undertake phlebotomy (take blood) procedures for housebound patients and within GP clinics
  • Be aware of how to dispose of sharps/clinical waste in accordance with GG&C policy

Knowledge, training, qualifications and/or experience required to do the job:

  • SVQ3 in Healthcare (or ability to demonstrate equivalent knowledge gained through similar experience)
  • Evidence of significant experience in a care environment
  • Can demonstrate significant community experience
  • Well‑developed communication skills; ability to work on own initiative and as part of a team
  • IT skills – word processing, email, internet
  • Ability to work to challenging deadlines
  • Ability to work on own initiative and as part of a team
