The role:

This is an exciting opportunity for a Nurse, AHP, or Pharmacist to join the Home First Response Team as an Advanced Frailty Practitioner (or Trainee) within Renfrewshire HSCP. The post supports the expansion of the service to a 7‑day model and is offered on a full‑time basis, with part‑time options considered for the first year. Candidates may apply on a fixed‑term or secondment basis (with prior line‑manager approval for secondment).

Working within a multidisciplinary team across acute and community settings, the post holder will play a key role in supporting people living with frailty to maintain independence, improve wellbeing, enable early discharge, and prevent avoidable hospital admissions. The role spans the primary–secondary care interface and contributes to a virtual network of Frailty Practitioners working across acute front‑door areas in Renfrewshire and Glasgow City HSCPs.

The post holder will be instrumental in strengthening communication and joint clinical decision‑making across teams and services, ensuring individuals receive the right care in the right place at the right time. As a peripatetic practitioner, they will work flexibly across acute and community environments to support the development of a sustainable 7‑day frailty service.   • Shift pattern for this post are 5 over 7 days


Key responsibilities:

  • Professionally and clinically leads on the development of frailty services in conjunction with colleagues throughout acute and community health and social care partnerships
  • Continually develops and implements changes within frailty services in-line with divisional and national strategic direction, incorporating patient / public involvement
  • Lead senior staff within the frailty team, providing managerial support and advice and collaborate with colleagues across traditional geographical, service and professional boundaries to build on existing good practice
  • Demonstrates clinical leadership and be readily available to offer expertise to all members of the multi-disciplinary team
  • Is responsible for the management and delivery of the hub and spoke frailty model across both acute front door areas and across health and social care partnership services
  • Ensures Standards of Practice and Organisational Policies are integrated into the work of the frailty team and feedback to the wider hospital and health and social care partnership teams
  • Responsible for ensuring the frailty service responds to national initiative and policies within the clinical governance framework, including clinical effectiveness, evidence based practice, managing clinical risk and research and development issues



Knowledge, training, qualifications and/or experience required to do the job:

  • Registered Nurse or Allied Health Professional with significant comprehensive post-registration experience within a healthcare setting, some of which must be at a senior practitioner level
  • Masters level qualification, or working toward, in a relevant specialist field
  • Evidence of experience (ideally multi-professional approach) of working with patients with complex needs
  • Expert knowledge in the field of older adult care and unscheduled care
  • Advanced knowledge and experience of a wide range of medical conditions
  • A working knowledge of NHS and Professional Standards, Policies, Procedures and Clinical Guidelines
  • Excellent verbal and written communication skills
  • Evidence of professional and service development
  • Expert delivery of presentations and teaching experience
  • Flexible and positive approach to work in a challenging and demanding environment
