An exciting opportunity has arisen within the West Dunbartonshire’s Older Peoples Community Mental Health Team. The post holder will be responsible for contributing to the care and support of older people affected by mental health referred to our service by their GP and other health services.
The post holder will have to undertake duties without direct supervision which includes working in partnership with patients and their relatives, liaising with third sector agencies, line managers and other staff within the Older Peoples Community Mental Health team and wider HSCP. Experience in Older adults Mental Health is desirable. The post holder will work alongside a multidisciplinary team including Psychiatrists, Psychologist, Occupational Therapist, Physiotherapist, Health Care Support Workers and Alzheimer Scotland Post Diagnostic Support Workers.
The post holder provides a service to a large geographical area for patients who are experiencing mental health problems and also has a key role in promoting and developing the role of the Community Psychiatric Nurse. The post holder will provide high quality, culturally sensitive and effective specialist nursing to older people with mental illness living in the community by working in partnership with service users, care providers and other agencies. The post holder will be expect to identify patients with complex needs requiring referral to psychiatric inpatient services, other agencies/teams both primary & secondary health care and voluntary agencies. The post holder will have the opportunity to support Student Nurses in practice education placements.

- Qualifications: Registered Mental Health Nurse (RMN) with valid UK NMC registration
- Experience of working with older adults or in a community mental health setting (desirable not essential)
- Skills: Assessment, care planning, risk management, communication, and teamwork
- Other: Full UK driving licence and access to a car is desirable as post holder would be expected to travel to appointments within the locality.
Duration, Location, and Working Pattern:
- The post holder will join the Cairnmhor Older Adult Mental Health Team within Dumbarton Joint Hospital
- The West Dunbartonshire OACMHT cover a wide geographical area including Dumbarton, Balloch, Helensburgh, Luss, Cove & Arrochar
- The shift pattern for this post is Monday to Thursday 09:00 - 17:00, Friday 09:00 – 16:30 – currently reviewing reduced working week for board wide reduction to 36 hours per week
Key responsibilities:
- Undertake comprehensive mental health and memory assessments, risk assessments, and care planning for older adults referred to the service
- Monitor effectiveness and side effects of medication prescribed by the mental health team and act accordingly when issues are raised
- Deliver person-centred, evidence-based interventions in line with individual care plans
- Developing therapeutic relationships with patients and carers thereby ensuring that care needs are identified and met within a participative framework which recognises the principles of equality, dignity and respect and which takes into account spiritual and cultural diversity
- Conduct the majority of appointments within patients’ own homes, ensuring safe and responsive care
- Monitor, review, and evaluate care plans to ensure ongoing effectiveness and appropriate discharge planning
- Supervise junior staff in all areas of nursing practice, completing appraisals and personal development plans as appropriate
- Contribute to weekly MDT meetings, actively sharing observations and clinical updates to support collaborative decision-making
- Work collaboratively with other professionals including medical staff, psychologists, occupational therapists, physiotherapists, and social services to deliver integrated care
- Initiate and develop new ideas to improve patient care and provide advice relating to changing community based mental health services
- Maintain accurate and timely clinical documentation
- Work autonomously in the community without direct supervision, while accessing advice and support from Band 6 colleagues and senior clinicians as required
- Engage in regular clinical and managerial supervision and attend mandatory training
- Adhere to the NMC Code of Professional Conduct and local policies and procedures
- Participate in audits, service development initiatives, and quality improvement projects as directed
Knowledge, training, qualifications and/or experience required to do the job:
- Registered Mental Health Nurse (RMN) with live UK NMC registration
- Evidence of basic professional development
- Basic knowledge of Dementia Strategy and how this important for working with patients who have diagnosis of a dementia
- Evidence of effective leadership skills applicable with the Band 5 role
- Evidence of excellent team working skills & ability to work independently
- Ability to work using own initiative and without direct supervision
- Effective communication skills
- Effective listening and interpersonal skills
- Full Driving Licence & access to a car is desirable as the post holder requires to drive to patients home for appointments
