The Role
You will work in partnership with Social Work services and Primary Care Pharmacy teams to enhance medicines management support across the community. A key aspect of the role involves delivering structured training and ongoing support to Care at Home staff to improve knowledge, skills and safe practice in medicines management.
You will also provide direct support to service users identified by health or social care professionals as experiencing medication-related challenges. This includes individuals recently discharged from hospital, as well as those living at home who require additional support to manage their medicines effectively.
The post holder will conduct home visits to:
· Assess patients’ ability to manage medicines independently
· Review adherence and concordance
· Provide practical solutions and medication support aids
· Work collaboratively with carers and support staff
· Refer to GP-based pharmacists for medication or polypharmacy reviews where appropriate
· Contribute to reducing inappropriate prescribing and polypharmacy
This role is closely aligned with North Lanarkshire Council Social Services and Primary Care Prescribing Support Teams, ensuring a coordinated and seamless approach to medicines optimisation across the Health & Social Care Partnership area.
In this key role, you will:
To collect and collate patient related prescribing and clinical data from practice computer systems and/or patients’ notes
To identify and highlight areas where patient prescribing management requires intervention and to make relevant recommendations to the practice in support of reviews related to priority areas.
To create reports and letters for GP surgeries detailing work undertaken, outcomes and recommendations e.g. audit data, patient data collection and staff training.
To implement agreed prescribing changes to ensure high quality and cost effective prescribing
Support practices in performing audits related to medicine use and patient reviews in line with locality and practice priorities
What You'll Bring:
Pharmacy technician, NVQ3 or BTEC in pharmaceutical sciences or equivalent
Registered with the General Pharmaceutical Council
Experience and knowledge of medicines management
Ability to work with limited supervision
Awareness of legislation and good practice related to prescribing, storage and handling of medicines
Clinical understanding of medicines use
A driving licence is required; this must be a full UK/EU/EEA licence.
From 1 April 2026, the Agenda for Change full-time working week will reduce from 37 hours to 36 hours. Part-time working hours will be reduced on a pro-rata basis. A corresponding increase in the hourly rate will apply, ensuring that overall pay remains unchanged.
