The role:

The Significant Adverse Event Review (SAER) Support Team has been supporting SAERs commissioned within Mental Health Services since 2022. The Team are looking to expand the team in order to provide support to additional areas and specialities within Mental Health Services.
The post holder will play a key role across Mental Health Services; making a substantial contribution to ensuring that clinical care and services are high quality, safe, effective and person centred.

The post holder will be required to use initiative and resource while exercising a high degree of confidentiality, diplomacy and tact whilst providing an effective and efficient administration and support role to SAER review teams in order to facilitate the timely completion of reviews in-line with Board policy. Workload will be generated by the SAER Clinical Support Lead. The post-holder will work within pre-determined procedures and parameters and is required to act independently, prioritising work with periodic review from the SAER Clinical Support Lead. Duties will include:

The management of a portfolio of SAERs: maintaining regular contact with review teams, establishing and maintaining patient/family/staff contact, organising meetings and interviews, minute taking and transcribing, completion of aspects of the SAER report and templates such as timelines and patient history.
Acting as liaison and primary contact role to all staff groups, patients, relatives, carers and external agencies who are involved in SAERs

Providing advice and support to Incident Review Groups (IRG) and Adverse Event Oversight Groups (AEOG)

  • Location based in Brand Street Resource Centre
  • Permanent Contract
  • Full-time position (36 hours per week)
  • Shift pattern for this post are Monday to Friday - 09:00 to 17:00


Key responsibilities:

  • Provide a high quality administrative service to support the SAER process and review teams.
  • Provide and receive complex, multifactorial clinical quality information; which involves the production of written and verbal reports for key stakeholders.
  • The post holder has significant discretion to work within a set of defined parameters
  • Update patients and family of SAER progress, expected completion dates and delays
  • Respond to queries from patients/family/staff re SAER policy, involving review team and management teams where appropriate
  • Obtain feedback from patients/family on the SAER process and ensure this is communicated to the Clinical Risk Team
  • Maintain and update information on SAERs in relation to review progress
  • Collate relevant information from patients electronic record as advised by review team
  • Request copies of paper patient records if appropriate from medical records / patients GP
  • Maintain SAER file of all documentation
  • Is aware of the requirement to maintain the confidentiality of both personal data and patient/client files also applies appropriate systems to preserve the confidentiality of information, following the requirements of the Data Protection and Information Security Policies.

Knowledge, training, qualifications and/or experience required to do the job:

Essential Criteria:

  • Excellent communication and organisational skills
  • The ability to work independently; managing time and task effectively
  • Experience of incident investigation and root cause analysis
  • Experience of extracting and summarising complex data from information systems
  • Experience of minute taking and transcribing of minutes
  • A working knowledge of Office 365; particularly Word, Excel, Teams and Outlook

Desirable Criteria:

  • A working knowledge of patient information systems such as EMIS and Clinical Portal
  • Experience in undertaking Significant Adverse Event Reviews (SAERs)
  • Experience of having ‘difficult conversations’ with staff and public
