The Role
The post holder will provide an accessible, person centred effective school nursing service for vulnerable school aged children who have been identified as being at risk and requiring additional support. This will be achieved by working in partnership with children, young people, families, carers and partner agencies.
A continuous focus on prevention, early intervention and support will prevail and concentrate on the ten priority areas under overarching themes of vulnerable children and families, mental health and wellbeing, and risk taking behaviours.
The post holder will be responsible for a caseload and function as an autonomous practitioner by virtue of in-depth knowledge, expertise, experience and proficiency.
In this key role, you will:
- Assess and identify vulnerability within individual/family/community needs, provide person centred care and, together, plan, implement and evaluate programmes for health improvement which are evidence based and make full use of all resources using a multi-agency approach to improve the health of the school aged population.
- Support children/ young people during periods of transition, e.g. school to school.
- Be responsible for making professional judgements and decisions with support from Team Leader relating to sharing of relevant and proportionate information about individual children/families in accordance with key information sharing guidance, policy and legislation.
- Have an understanding of the determinants of health in order to promote the wider public health role of school nursing.
What You'll Bring:
- Comprehensive knowledge of clinical guidelines and standards within the speciality / area of practice.
- Specialist knowledge and experience of relevant conditions appropriate to areas of responsibility.
- Ability to communicate and engage effectively with looked after children, ensuring that they have the opportunity to participate in decisions affecting them as appropriate to their age and ability.
- Awareness of national policy and legislation relating to children and young people.
- A driving licence is required; this must be a full UK/EU/EEA licence.
From 1 April 2026, the Agenda for Change full-time working week will reduce from 37 hours to 36 hours. Part-time working hours will be reduced on a pro-rata basis. A corresponding increase in the hourly rate will apply, ensuring that overall pay remains unchanged.
