The Role

To lead, develop, co-ordinate and implement a safe and effective specialised clinical pharmacy service to a specific or general patient population, including prescribing as an independent prescriber

To manage and provide leadership to identified pharmacists and technicians in the HSCP.

Provision of professional and highly specialist prescribing advice to the extended Primary Care team


In this key role, you will:

To provide leadership, management and direction to pharmacists and technicians providing prescribing support and clinical pharmacy services within GP practices or other healthcare settings

Responsible for provision of pharmaceutical care for an identified caseload of patients in primary care sites. As an autonomous clinician using independent prescribing and clinical judgement, apply expert clinical knowledge to analyse complex patient information and make clinical decisions for optimal outcomes.

To provide specialist advice involving clinical interpretation of evidence and clinical judgement to medical staff and the wider multidisciplinary team to inform the treatment of individual patients with complex needs. This includes advice on optimal drug regimens, avoidance of adverse drug reactions, drug interactions and contra-indications ensuring compliance with medicines legislation.

Report and review adverse clinical incidents, identifying causes of medication errors and use expert clinical knowledge to provide advice on strategies to minimise medicine use risks


What You'll Bring:

Masters Degree of Pharmacy or equivalent

Registered with the General Pharmaceutical Council (1 year pre-registration practice and competency assessment with examination to enter the register)

Relevant Post Graduate pharmacy qualification (or equivalent) or equivalent specialist knowledge gained through experience obtained in a primary care or a clinical setting

Independent prescriber or already undertaking the course

A driving licence is required; this must be a full UK/EU/EEA licence.
