We seek a Senior Clinical Pharmacist to join the Medicine for the Elderly Clinical Pharmacy Team and help transform pharmaceutical care across traditional boundaries. The Medicine for the Elderly clinical pharmacy team plays a pivotal role in enhancing care throughout the patient journey and, across interfaces of care. Older patients often present with complex health conditions and multimorbidities with polypharmacy requiring specialist expertise in medication management and optimisation.
As a Senior Clinical Pharmacist, you will provide pharmaceutical care to a case-load of patients across Ninewells Hospital and the Angus community hospitals to ensure medicines are used safely, effectively and tailored to the specific needs of frail and older adults.
Key responsibilities
- Undertake comprehensive medication review to optimise therapy and improve patient outcomes
- Identify and manage potential medication-related harm
- Collaborate closely with multidisciplinary teams across the patient pathway
- Contribute to service development and quality improvement initiatives

You should have previous experience of working within a clinical environment. Training will be provided and you will be supported to develop a portfolio and seek accreditation in line with the RPS Core Advanced Pharmacist Curriculum. A commitment to continuing professional development is essential. You must be registered with the General Pharmaceutical Council and possess a prescribing qualification or be prepared to work towards one.
