The role:

As a Heart Failure Clinical Nurse Specialist, you will be an advanced nurse practitioner with additional specialist knowledge and expertise in managing Heart Failure. You will work as an autonomous practitioner, optimising evidence-based pharmacological and non-pharmacological treatments, providing education to patients, carers, and colleagues, reducing hospital readmissions, facilitating safe and timely discharge, and improving patients’ quality of life.

You will provide specialist care to a designated clinical service with a complex caseload, including patient assessment, treatment, and discharge from care. You and the team will competently perform the necessary technical, invasive, and physical aspects of care for this patient group.

You will also contribute to consultancy, teaching, audit, research, and service development, ensuring service objectives meet the evolving demands of delivering specialised Heart Failure care. Your role combines direct clinical care with leadership, teaching, and research responsibilities, ensuring the development, implementation, and evaluation of evidence-based care programmes.

You will act as an expert resource for the clinical management of Heart Failure patients and will be certified or working towards Non-Medical Nurse Prescribing within this specialist service.

Duration, Location, and Working Pattern:

  • This is a full time post (36)
  • Working hours - Monday to Friday 0800 to 1700
  • This post will be based within Queen Elizabeth University Hospital and Gartnavel General Hospital.

Key responsibilities:

In this role, you will assess patient needs, plan and implement programmes of care, and evaluate outcomes.

You will provide education and support to patients, carers, and nursing and multidisciplinary colleagues, and optimise evidence-based treatments to improve quality of life and reduce hospital readmissions.

You will facilitate discharge planning and provide palliative care when required, manage telephone advice lines, triage incoming calls and patient referrals, and liaise with hospital clinicians and GPs.

You will act as a clinical leader, contributing to consultancy, teaching, research, and service development, while maintaining high standards of patient care in line with clinical governance and professional guidelines. You will uphold confidentiality, patient-centred care, and culturally competent practice, and ensure your TURAS and Statutory/Mandatory Learnpro training is kept up to date, including relevant educational modules

Knowledge, training, qualifications and/or experience required to do the job:

  • NMC Registered Adult Nurse with experience as an Advanced Nurse Practitioner.
  • Current specialist knowledge of Heart Failure management.
  • Experience managing a complex caseload within agreed protocols.
  • High-level interpersonal and communication skills, with the ability to work independently.
  • Experience in clinical governance, audit, research, and service development.
  • IT skills, including e-forms, Trakcare, and portal.
  • Valid driving licence.
