Band 4 - Assistant Practitioner - Hospital To Home, Edinburgh Partnership
Motivated and experienced Band 4 Community Assistant Practitioner to join Edinburgh Hospital to Home team to provide care to patients requiring package of care provision for support with activities of daily living between 0800-2200, in their own homes within the city of Edinburgh. The successful candidate will also provide nursing interventions, such as catheterisation, injections and contribute to diabetes care to patients in line with Band 4 Community Assistant Practitioner role as well as supporting junior staff, the successful candidate will also be expected to support the nursing team to deliver safe, effective clinical care.
We deliver social and clinical nursing care to patients requiring palliative and end of life nursing and social care with an 8 week or less prognosis to support hospital discharge or prevent admission to hospital / hospice. Therefore, the successful candidate must have excellent communication skills and experience of working at Band 4 Assistant Practitioner level, as many of our patients have complex nursing needs and are approaching end of life.

As from 1/4/26, the Agenda for Change full-time working week will be reducing from 37 to 36 hours per week. Part time hours will be reduced pro-rata. However, there will be a corresponding increase in the hourly rate so that pay will not be affected.
