The Role
To provide patient focused quality pharmaceutical care and to assure an efficient and timely supply of pharmaceutical advice and medication within secondary care setting.
AND / OR
To assist in the delivery of pharmacotherapy and prescribing support within a Locality in primary care setting
Please note this is a fixed term post for 12 months. Where a post only has temporary funding and an existing member of NHS Lanarkshire staff wishes to apply, this will be treated as a secondment. The employee MUST already have written agreement from their line manager to be released on a secondment before applying for the post.
In this key role, you will:
To develop competence and to deliver patient-centred pharmaceutical care to patients in various specialities including acute medicine, acute surgery, emergency care and / or primary care.
To participate in consultant ward rounds and multidisciplinary team meetings as directed by senior pharmacists.
To deliver patient education to ensure the safe and effective use of medications and devices
To deliver clinical care to patients by assessment of their pharmaceutical care needs and developing and actioning a care plan.
To review and analyse both medicine and patient information and provide specialist advice on drug administration and changes to therapy accounting for individual patient needs and clinical condition and local management protocols
What You'll Bring:
Masters of Pharmacy Degree
Registered member of General Pharmaceutical Council
Independent Prescribing qualification or undertakes to complete within reasonable timescale.
Ability to work under pressure.
Able to plan, prioritise and organise workload
Experience in patient information/education
Ability to effectively monitor patients’ therapeutic regimens in order to ensure effectiveness and reduce toxicity
Able to work under their own initiative and as part of a team
