Position:

Advanced Pharmacist Prescriber in Assertive Outreach

This is an exciting opportunity for a highly motivated pharmacist prescriber to be involved in the delivery of a novel service to make a difference for people in North Ayrshire who face severe and multiple disadvantage, such as homelessness, substance use, overdose, and involvement with justice services. Based in Caley Court, Stevenston, this post is principally based within North Ayrshire and will involve flexible working across the geographical area.

Hours and Shift Pattern:

Flexible working is available and required to deliver the position over 22.5hours a week.

This post is offered on a fixed term basis for 12 months.

What you will do:

The pharmacist prescriber will work alongside a number of individuals, teams and supporters within North Ayrshire. The post holder will provide outreach engagement into the heart of North Ayrshire’s communities, seeking contact with people with severe and multiple disadvantage in need of support and offering holistic health and social care. The pharmacist prescriber will offer assessment and intervention to address physical, mental health and substance use needs, either through prescribing or through referral into appropriate services and agencies available within the area. The post holder may also need to offer help with welfare benefits, social prescribing, advocacy, accommodation, vocational training/opportunities, and accompanied attendance at appointments. The post holder will work to build supportive therapeutic relationships, increase accessibility of health and social care, reduce overdose risk factors, improve health, increase societal connectedness, meet wider social care and recovery needs, and reduce offending.

Knowledge, training and/or experience required to do the job:

The nature of the role will require a pharmacist prescriber with a willingness to engage with a population who may be disenfranchised and untrusting of authority. The pharmacist role will require a level of confidence to diagnose undifferentiated conditions, prescribe and provide care for immediate issues which are identified, as well as to progress referral and contact with other service providers. The role will require a person with the ability to build positive and compassionate relationships, and someone who is outgoing and can quickly develop a level of rapport with people who may need support. The post holder will have a very high level of autonomy and will be expected to operate at an advanced level, taking responsibility for clinical and care decisions made as well as supporting others to act on behalf of those for whom they advocate. Experience of working with those experiencing homelessness and in community outreach is highly desirable. Advanced clinical examination and consultation skills and experience of providing undifferentiated care in uncertain clinical settings are considered essential.
