The Role
As a member of the wider Primary Care Team the post holder will work towards meeting the health needs of the local population, develop and implement evidence based practice and actively liaise with colleagues in secondary care and specialist teams with a view to agreeing common aims and clinical approaches and pathways. The nurse clinician will independently provide primary care advice, assessment, treatment and support to the patients attending the Primary Care Emergecny Centres. The post holder will assess patients presenting to the Out of Hours service, without first seeing a GP, treating, discharging or referring each patient as clinically appropriate. In addition the post holder will be responsible and accountable for the leadership and management of the nursing team and other staff groups where applicable, linking into the management structure of the OOH’s Service.
In this key role, you will:
- Responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care for patients to ensure delivery of a high standard of care.
- Utilise advanced specialty assessment skills to facilitate intervention for patients.
- To maintain effective communications with patients / carers and other members of the multidisciplinary team to ensure that the appropriate information is shared, and patient needs are met.
- Co-operate with the Service Manager/Lead Nurse to set, monitor and evaluate standards of care within defined policies, procedures and protocols of the service, CHP, and division and Professional Regulatory body, to ensure high quality service.
What You'll Bring:
- Registered Nurse with at least 5 years post registration experience.
- Educated to/ working towards or operating at degree level.
- Further education/evidence of continuous professional development in area of specialty including study days/courses/post graduate qualification.
- The post holder will possess excellent team working/leadership skills and have the ability to motivate others and work using own initiative.
A driving licence is required; this must be a full UK/EU/EEA licence. delete if not required
