The Role
- To lead, develop, deliver and evaluate clinical pharmacy services to a specified patient population or directorate/clinical specialty in accordance with the objectives set between the on site Hospital Pharmacy Manager and the Principal Clinical /Deputy Pharmacy Manager. In primary care objectives may be set by the Lead Pharmacist Primary Care or Lead Pharmacist Advanced Clinical Services
- To act as a role model and provide a consistently high quality pharmaceutical service.
In this key role, you will:
- To lead clinical pharmacy services for patients admitted to, or treated within, area of responsibility in accordance with the priorities set by the post holder, Hospital Pharmacy Manager, Lead Pharmacist Primary Care or Lead Pharmacist Advanced Clinical Services, and Clinical Director in order to ensure safe, effective and cost efficient use of medicines in these patients.
- To identify and prioritise clinical pharmacy services to the directorate/clinical specialty.
- To monitor clinical pharmacy services for specified patient population or directorate / clinical specialty in accordance with the objectives set by the post holder and Hospital Pharmacy Manager and Principal Clinical/Deputy Pharmacy Manager or Lead Pharmacist Primary Care or Lead Pharmacist Advanced Clinical Services.
- To demonstrate expert clinical knowledge with advanced level of clinical reasoning and judgement.
What You'll Bring:
- Masters degree in Pharmacy.
- Registered to practice with the General Pharmaceutical Council.
- Continuing Professional Development as detailed by the General Pharmaceutical Council (mandatory requirement).
- Ability to monitor patient therapeutic regimens to ensure effectiveness and reduce toxicity.
