The role:

This role involves becoming part of a large, multidisciplinary and experienced community mental health team. The post holder will contribute to the care and support of individuals referred into the service, working collaboratively with patients, carers, and other professionals. The role requires delivering care based on agreed plans using a strength-based, recovery-focused approach that promotes self-management and active involvement from the individual.


Duration, Location, and Working Pattern:

Permanent post, Full-time

Shift Pattern - Monday- Friday

Key responsibilities:

  • Deliver care and support in line with agreed care plans using a recovery-focused approach
  • Work independently while adhering to legislative frameworks, standards, and clinical policies
  • Collaborate with patients, carers, families, and multidisciplinary teams
  • Support individuals with complex mental health needs and associated risks
  • Provide assertive care and manage higher levels of risk where required
  • Participate in caseload management, line management supervision, and clinical supervision
  • Build therapeutic relationships as a named nurse or key worker
  • Deliver care in accessible environments suited to patient needs
  • Apply a stepped or matched care approach based on individual assessments

Knowledge, training, qualifications and/or experience required to do the job:

  • Understand and apply a strength-based, recovery-focused model of care
  • Work within relevant legislative frameworks, clinical policies, and agreed standards
  • Be familiar with multidisciplinary and integrated care approaches
  • Engage in ongoing clinical and caseload supervision as part of professional development
  • Understand the wider mental health service system, including primary care, acute services, and specialist services
  • A driver's license is not essential but is desirable.
