The Role

The service provides specialist palliative care nursing across Lanarkshire which has a population of approximately 655,000 people, 92 GP practices, 3 community hospitals and numerous nursing / care homes.

Referrals are made for patients who have complex care needs requiring the support of a specialist palliative care clinical nurse specialist, working as part of a multidisciplinary team

As an autonomous practitioner the post holder will function as a specialist resource to primary care teams in a defined locality and to the wider multi-disciplinary team which includes Marie Curie, community nursing services, acute hospital services, Hospices and other stakeholders.

The post holder will assume delegated responsibility through referral for the specialist assessment, planning, implementation and evaluation of patients care needs.

The post holder will contribute specialist palliative care knowledge to the development and implementation of education programmes, local guidelines and policies.

In this key role, you will:

• Demonstrate competence in their specialist area and across all four pillars of practice.
• Have the autonomy to take responsibility for specialist holistic assessment, informed decision making and treatment of patients with specialist palliative care needs.
• Be expected to work as part of a multidisciplinary team contributing to case management presentations and act as a skilled advisor or resource for others.
• Demonstrate effective communication skills and have the ability to initiate and lead complex conversations around symptom management, future care planning and end of life care.
• Participate in audit and quality assurance programmes, promoting evidence based practice.
• To provide bereavement support for carers and specialist advice to other professionals and agencies on bereavement related issues.
• Work collaboratively with the Band 7 Team Leaders/ACNS’s and Nurse Consultant in the Specialist Palliative Care Team to provide a high quality nursing service.

What You'll Bring:

• Registered Nurse or AHP with valid NMC / HCPC Registration
• Educated to Degree Level
• Post graduate qualification at a minimum of Level 10 (Hons) or working towards
• Expertise within speciality / area of practice that demonstrates the required breadth of knowledge required to lead safety, effectively and efficiently.
